Start the console session.
Plug in serial cable to console port of device from PC COM1 port
Open PuTTY and select "Serial" connection type, leave baud rate (Speed) and other settings as default.
Plug power into device and if a power button exists, turn it on.
Reset Password.
At the login prompt type maintainer and type bcpb followed by the Serial Number of the device for the password. You only have 14 seconds to do this:

PHSTDCDFW02 (global) # exit
Erase configuration.
Login as admin with Pa$$w0rd as the password:
PHSTDCDFW02 login: admin

Type config global then exec factoryreset and y to confirm:

There is a backup system with a configuration that also needs erasing.
Load backup system.
When prompted press a key for the configuration menu:

[G]: Get firmware image from TFTP server.
[F]: Format boot device.
[B]: Boot with backup firmware and set as default.
[I]: Configuration and information.
[Q]: Quit menu and continue to boot with default firmware.
[H]: Display this list of options.
Enter Selection [G]:
Type b to boot the backup and set it as default:
Enter G,F,B,I,Q,or H: b
Loading backup firmware from boot device...
Reading boot image 1483900 bytes.
Initializing firewall...
System is starting...
Starting system maintenance...
Erase the backup.
Repeat steps 2 and 3 on the backup.
Copy all the output from the session into your records.
Right-click PuTTY's titlebar, from that dropdown menu click on "Copy All to Clipboard"
Open the file of record certifying the deconfiguration of this device and use CTRL V to paste all the output from PuTTY into that file. Formatting the text with a fixed-width font helps it look more readable.
End the session.
Close PuTTy.
